Disclosures

1. This is not insurance.
2. This discount program provides a 30 day cancellation period.
3. Hearing service is not available in Colorado, Missouri, Texas or Florida.
4. This ACBA membership package is not available in the states of Louisiana, Utah, Oregon, Washington, Connecticut, Kentucky or Maine.

Terms and Conditions

1. ACBA Membership includes both insured and non-insurance benefits and services. ACBA is not
   an insurance company. It is an association that enrolls members.
2. The insurance is provided under a group policy as added separate benefits made available
   to active members of ACBA.
3. ACBA provides savings to its members on services through a number of sources. The current list of benefits may be modified through additions or deletions at its discretion. Healthcare
   professionals providing healthcare services at managed care pricing receive no reimbursement from ACBA.
4. ACBA does not sell insurance. ACBA membership reduces healthcare costs to its members
   through networks of providers.
5. Membership in ACBA will automatically terminate if a member’s dues are not paid on a timely
   basis. Returned checks or NSF notices on bank drafts for payment of the membership dues is
   deemed to be evidence of nonpayment by a member.
6. Membership canceled within the first 30 days of the enrollment date is eligible for a refund if a
   signed cancellation request is sent to ACBA.
7. Membership is effective when payment is processed in home office. Membership is renewable
   at the Member’s option. Non-payment will result in cancellation of membership. A member may
   cancel at any time by written notice to ACBA.

Member Agreement
By signing the enrollment form, Member expresses desire to become a member of American Combined Benefits Association. Membership dues may be changed for all members, but not individually, upon 30 days notice. The benefit information contained herein is a brief summary only and subject to all provisions, limitations and exceptions set forth in the membership packet.
